Medicare Basics

Medicare itself has four parts: A, B, C and D. The two Original Parts are Part A, which is your hospital insurance, and Part B, which is your outpatient insurance.

Like any other insurance, Medicare doesn't cover every dime of your healthcare services. You will have deductibles to pay on both part of Medicare before your benefits begin each calendar year. Then Medicare covers about 80% of your outpatient medical services. You are responsible for the other 20%. There is no cap on what you might spend, so most people buy Medigap coverage to fill in the gaps.

Medigap plans are offered in all 50 states. In most states, there are ten standardized plans to choose from, labelled A - N. (Minnesota, Wisconsin and Massachusetts Medigap plans are structured differently.) Each standardized plan covers the same set of benefits no matter which insurance company you purchase your Medigap policy from.

Plans F and G have been very popular in recent year. Medigap Plan F covers all of the gaps in Medicare. You show your provider your Medicare card and Medigap Plan F card, and you will not owe any copays or deductibles at all - not for doctor visits and not for expensive things like surgeries, either. All Medicare covered services get first dollar supplemental coverage from Plan F.

Plan G functions just like Plan F except you will pay the Part B deducible once per year. Premiums for Plan G are often much lower than for Plan F so it often makes sense for your wallet to purchase Plan G anyway.

Medicare Part D

Medicare and Medigap cover medications administered in a hospital or clinical setting, but they do not cover outpatient retail prescription drugs. In 2006, Medicare Part D was created, and it is a pharmacy card that you can purchase to help you lower the costs of your medications.

While Part D is voluntary, you should consider strongly to enroll. There is a late penalty for not enrolling when you first are eligible, and that penalty grows with time. Also, you can only join Part D at certain times of the year, so if you fail to enroll, you could find yourself needing a prescription without being able to get coverage until the next Annual Elect Period.

Part plans are offered by various insurance companies in each state. You must be enrolled in either Part A or B and live in the plan's service area to eligible to get one. Once you enroll, you are typically locked in for a full calendar year. You can only make changes in the fall during the Annual Election Period or if a special circumstance qualifies you for a mid-year Special Election Period, such as moving out of state, or qualifying for Medicaid.

Some people to choose to get their Part D bundled with their Medicare A and B benefits through a Medicare Advantage plan. These are private plans that pay instead of Medicare and are also referred to as Medicare Part C. Advantage plans typcially have networks of providers, so it's important to make sure your providers are in the network before joining one.
